If you are a new patient to our office, we are pleased to announce that we are now doing electronic medical record keeping, and ask that all information be filled out online. Click the link here to enter our portal.   You will need to contact us during business hours to get the security code to access the portal. You will then be able to set up a username and password.

You must use either Firefox or Google Chrome Browser to use the portal.  The older Internet Explorer will not work properly.  For questions please call our office. 716-839-1700
